HOW TO REGISTER
Open the Registration Form in a separate browser window, then follow the steps listed below:
Step 1 - Fill in the form on your computer screen with your all of the required information.
Step 2 - Before you sign please read the "Refund Policy" at the end of the form, then print, sign and date it.
Step 3 - Mail the registration form and check to the applicable address:
