REGISTRATION INSTRUCTIONS

Step 1 - Fill in this form on your computer screen with your all of the required information.

Step 2 - Read the Refund Policy at the end of this form, then print, sign and date it.

Step 3 - Make out a check payable to "Retirement or WHAT NEXT" for the applicable program fee.

Step 4 - Mail the registration form and check to the applicable address:

If you should need to cancel your registration at any time, the following rules apply:

A $50 processing fee is charged for all cancellations.
A full refund of the program fee (minus the processing fee) will be made if a cancellation is made prior to 3 weeks before the program begins.
A refund of half of the program fee (minus the processing fee) will be made if a cancellation is made within 3 weeks before the program begins.
No refund will be given after the program begins.
